Please complete the secure self-referral form below to request postpartum care. We’ll review your information and respond within 2-3 business days.​

• All information shared is confidential and used only to coordinate your care.

• No referral required for full postpartum care (which includes feeding support) from after hospital discharge through 6 weeks postpartum. Ideally, we would love to meet you in your third trimester before your baby arrives!

• A referral is required if you only need lactation/feeding consultation and not full postpartum care. This can include prenatal consultation of up to 3 hours and postpartum consultation of up to 4 hours, both covered by MSP.  Referrals can be made by any midwife, doctor, nurse or social worker via the Professional Referral Form.
